Community Integration Program

Community integration is a way to ensure that all people have access to the full spectrum of social, educational, and professional opportunities. These opportunities are embedded within the existing community, rather than segregated.

Specialized Consultative Services

Specialized Consultative Services offers expertise, training and technical assistance in a specialty area to assist family members and support staff in helping individuals with intellectual and/or developmental disabilities. Under this model, family members and caregivers are trained by licensed professionals on how to carry out therapeutic interventions to improve the individual's quality of life.

Nursing / Respite Program

Respite is a break for unpaid caregivers who provide ongoing care for an individual of any age who needs help, care, or supervision due to disability, chronic condition, or other special needs.

Day Services Program

Covenant Case Management Services is excited to announce the opening of our day programs. The Covenant Dream Centers are in Matthews and Albemarle. We are looking at expansions within the next year to various counties within North Carolina. We provide day program services to people with intellectual disabilities. We are funded through the North Carolina Innovations Waiver and B3 Medicaid services. Our day support services are open to people 18+ years of age. The Covenant Dream Centers aim to reinvent the idea and stigma associated with historical day programs. We provide an innovative and fun environment where individuals will have a meaningful day and give them a safe place to feel supported and involved in programming, community connections, and assistance in facilitating their own personal growth. Each day program has community volunteering set into their schedules. Additionally, Music therapy comes into the program weekly, as well as art classes to provide our individuals with recreational enrichment activities. Each location has a program director who facilitates activities, outings, and community involvement. Every location has a fully equipped sensory room for individuals to enjoy, as well as a computer lab. We host computer classes for those who are interested. We have had so much fun since we opened in January, and we are excited for this journey!

The Albemarle Dream Center is located at 2000 West Main Street in Stanly County. This day program opened in January 2022. It is an open-concept space with a fun and colorful atmosphere.

The Matthews Dream Center is located at 1315 Matthews Mint Hill Rd. in Matthews, NC. This center opened 3.1.2022 and is set up with various rooms to give individuals personal choice of activities, seating, etc

Peer Support Program

Peer Support Services (PSS) are an evidenced-based mental health model of care that provides community-based recovery services directly to a Medicaid-eligible adult beneficiary diagnosed with a mental health or substance use disorder.
