Tailored Care Management (TCM)
Tailored Care Management (TCM) offers personalized support through a dedicated care manager who works closely with individuals to navigate the healthcare system and connect them with vital resources. The goal of TCM is to provide whole-person care management to help those we serve meet their health goals.
How It Works
Step
Enrollment & Assessment
Eligible individuals are automatically enrolled in Tailored Care Management, and the care management agency conducts an assessment to understand their unique needs and goals.
Step
Care Plan Development
Working collaboratively, the care manager and the individual create a personalized care plan that addresses healthcare, behavioral health, and social determinants of health needs.
Step
Resource Navigation & Coordination
The care manager assists with scheduling appointments, coordinating services between providers, and connecting individuals to community resources like WIC (Women, Infants, and Children), EBT stamps, transportation, and housing assistance.
Step
Ongoing Support & Monitoring
The care manager provides ongoing support, monitors progress, and adjusts the care plan as needed to ensure the individual receives the right services at the right time.
Who It's For
- NC Medicaid beneficiaries with serious mental illness (SMI).
- Individuals with substance use disorder (SUD).
- Those with intellectual/developmental disabilities (IDD).
- Individuals with complex health needs who require coordinated care.
Who Qualifies
