Tailored Care Management (TCM)
Tailored Care Management (TCM) offers immediate, personalized support through a dedicated care manager who begins assisting individuals as soon as they join. From addressing urgent needs to developing a long-term care plan, TCM focuses on whole-person care to help each person reach their health and life goals.
How It Works
Step
Enrollment & Assessment
Eligible individuals are automatically enrolled in Tailored Care Management by their MCO and then assigned to us. Our care management team conducts an assessment to understand their unique needs and goals.
Step
Care Plan Development
Through multiple in-depth assessments and in-person meetings, the care manager and individual work together to create a personalized care plan that supports healthcare, behavioral health, and social needs.
Step
Resource Navigation, Coordination & Care
The care manager works closely with the individual and their chosen support team—family, providers, and others—to coordinate services, schedule appointments, and connect them to vital community resources like dentistry, therapy, transportation, housing, and other support.
Step
Ongoing Support & Monitoring
The care manager provides ongoing support, monitors progress, and adjusts the care plan as needed to ensure the individual receives the right services at the right time.

- Those with intellectual/developmental disabilities (IDD).
- Individuals with complex health needs who require coordinated care.
Who Qualifies

Individuals enrolled in NC Medicaid who meet the eligibility criteria for IDD.
