Community Navigator

(b)(3) services are additional supports for individuals who have Medicaid. They are in addition to the services in the Medicaid state plan. These services focus on helping individuals remain in their homes and communities and avoid hospitalization or living in an institution. Cardinal Innovations Healthcare and the other LME-MCOs are able to offer these additional services as a result of the Medicaid waivers.
NC Innovations Waiver
The NC Innovations Waiver is a Federally approved 1915 C Medicaid Home and Community-Based Services Waiver (HCBS Waiver) designed to meet the needs of Individuals with Intellectual or Development Disabilities (I/DD) who prefer to get long-term care services and supports in their home or community, rather than in an institutional setting. To serve, inspire, and unite our community to become a sanctuary by using our gifts to create healing, restoration, and harmony.
The Medicaid Innovations Waiver supports Individuals with I/DD to live the life they choose. Waiver services are administered by a local management entity/managed care organization (LME and MCO) which facilitate services and oversee a network of community-based service providers. Individuals who receive waiver funding work with their team to develop a Person Centered Plan of Care and request the services and supports they need. NC Innovations provides many different services that you can use based on your Individual Support Plan (ISP). Covenant offers a variety of services to the families we support.

Supported Employment

It is for individuals with severe disabilities who need lifelong, ongoing support. An individual with vision loss who has additional, severe disabilities would, therefore, be eligible for supported employment. Most individuals with vision loss only need access to information and not direct support and therefore are not candidates for supported employment.
Example: Sally is blind, developmentally disabled, and is easily distracted. Because of these problems, she is not able to work without extensive training in job tasks and guidance to keep working. She works in the laundry department of a large hotel folding towels and sheets. Assistance is provided from a job coach in setting up her workstation each day and making sure that she stays on task. She folds laundry five days a week for three hours each day with pay at minimum wage.
Alternative Family Living
AFL (Alternative Family Living) Service: A residential service provided in the primary residence of an AFL Provider (couple or single person) who receives reimbursement for the cost of care.
Become an AFL!
As humans we need to feel loved, safe and valued. If you have room in your heart and home to provide these needs and you are interested in knowing more about being an AFL, contact us to speak with our intake specialist. For more info reach out on here or you can email us at ccms.info@covenanttoserve.com

Individuals with intellectual or developmental disabilities (I/DD) who are eligible for the NC Innovations Waiver or other Medicaid-funded services can qualify for AFL. An assessment by the local Managed Care Organization (MCO) helps determine eligibility based on the individual's needs and level of care required.

EOR & MCOs
In North Carolina, the Employer of Record (EOR) and Managed Care Organizations (MCOs) work together to ensure individuals with intellectual and developmental disabilities receive high-quality, self-directed services through the NC Innovations Waiver. The EOR is responsible for hiring, training, and managing caregivers while ensuring compliance with Medicaid guidelines. MCOs, such as Vaya Health, oversee the approval and coordination of services, providing support and financial oversight to maintain program integrity. This partnership allows individuals and families to have greater control over their care while ensuring access to essential resources and compliance with state regulations.
