Assessment & Service Planning
Every journey begins with a comprehensive clinical assessment to confirm medical necessity and identify personal goals. A person-centered plan is developed to support each individual’s path toward recovery, connection, and independence.

Service Delivery
Certified Peer Support Specialists (CPSS) provide one-on-one or group support in flexible settings such as home, community, or virtual sessions. Services may include peer counseling, emotional guidance, skill-building, and connecting participants to local disability counseling services and recovery resources.

All peer support services are supervised by a qualified professional (QP). Specialists collaborate with medical providers, therapists, and care coordinators to ensure continuity of care. Who Qualifies
To access Peer Support Services (PSS) through Covenant HeartLink:

The individual must be enrolled in NC Medicaid.
