Tailored Care Management (TCM)
Tailored Care Management (TCM) is a coordinated support service designed to bring clarity, connection, and compassion to individuals and families navigating complex health and behavioral needs. It ensures that every person receives the right care, at the right time, through a dedicated care manager who understands their journey and goals.
The goal of TCM is to improve quality of life through consistent communication, proactive care planning, and community-based support.
How It Works
Step
Enrollment & Assessment
Eligible individuals are automatically enrolled in Tailored Care Management through their Managed Care Organization (MCO). Once assigned to Covenant, a care manager connects with the individual and their support network to complete a comprehensive assessment. This first step helps identify health priorities, daily challenges, and personal goals.

Personalized Care Plan Development
Using the information gathered, the care manager develops a customized care plan. This plan is centered on whole-person care, addressing physical health, behavioral health, and social needs. Every plan is unique, reflecting the individual’s preferences and long-term aspirations.

Coordination and Connection to Resources
The care manager acts as a single point of contact, helping coordinate services across healthcare providers, community organizations, and family supports. This includes scheduling appointments, managing referrals, and connecting the individual to essential community resources such as therapy, dental care, transportation, and housing support.

Ongoing Support & Monitoring
Care doesn’t stop once a plan is created. The care manager provides ongoing communication, follow-up visits, and plan adjustments as needs change. This continuous support ensures each person receives consistent, reliable care and remains on track toward their goals.

Who Qualifies
To receive Tailored Care Management, individuals must meet the following requirements:

Be enrolled in NC Medicaid and meet the eligibility criteria for IDD

Be covered under Medicaid Direct or a Medicaid Tailored Plan (not the standard plan)
