Nursing Oversight in Residential Settings (Specialized Consultative Services (scs) Nursing)

Covenant ClinicalCare provides tiered nursing oversight tailored to the needs of individuals in residential care settings. It ensures both baseline health monitoring and specialized nursing support based on the level of medical complexity and service enrollment.

There are two levels of service:

  • Baseline Nursing Oversight – standard for all residential clients
  • Specialized Consultative Services (SCS) Nursing – enhanced oversight for clients with greater medical needs

 

How It Works

Step

Monthly Medication Administration Record (MAR) Review

A licensed nurse conducts a monthly spot-check of Medication Administration Records. Reviews for errors, omissions, and PRN documentation.

Medication Error Follow-Up

Nurses review any reported med errors and provide retraining or recommendations.

Change in Status Notifications

Staff must notify the nurse of ER visits, hospitalizations, extended time away from the residence, or any medication changes.

Quarterly Health Check-Ins

A general health review is conducted every quarter to identify emerging concerns or the need for SCS referral.

Who It's For

  • Baseline Oversight: All individuals in residential IDD settings, regardless of clinical complexity.
  • SCS Oversight: Individuals enrolled in Specialized Consultative Services due to elevated health needs or physician-directed care requirements.

What is included in the initial nursing assessment?

Within the first 30 days of referral, a licensed nurse performs a comprehensive evaluation. This includes:

  • A full medication review
  • Baseline vital signs
  • A review of current medical diagnoses
  • An assessment of ongoing nursing needs and caregiver support requirements
