Covenant Thrive
Specialized Disability Care Services
Covenant Thrive provides specialized care and therapy services that help individuals with intellectual and developmental disabilities live safer, more balanced lives. Our goal is to deliver personalized, medically necessary support through specialized consultative services that strengthen independence, improve daily living, and enhance overall well-being.
How It Works
Every person’s care journey is unique. Covenant Thrive begins with understanding each individual’s needs and tailoring care that fits their goals.
Step
Comprehensive Needs Assessment
A licensed therapist or nurse evaluates strengths, challenges, and health goals. This includes reviewing medical diagnoses, medications, and daily support requirements.
Step
Customized Care Plan
Based on the assessment, our team develops a person-centered plan that may include behavioral health therapy, dietary guidance, occupational or physical supports, and nursing oversight. Each plan follows state criteria for NC Innovations Waiver participants.
Step
Ongoing Consultation and Support
Our team offers continuous virtual and in-person consultative services, including behavioral, occupational, physical, dietary, speech, recreational, and nursing consultations. These sessions help ensure care plans remain effective and responsive to changing needs.
