Comprehensive Needs Assessment
A licensed therapist or nurse evaluates strengths, challenges, and health goals. This includes reviewing medical diagnoses, medications, and daily support requirements.

Customized Care Plan
Based on the assessment, our team develops a person-centered plan that may include behavioral health therapy, dietary guidance, occupational or physical supports, and nursing oversight. Each plan follows state criteria for NC Innovations Waiver participants.

Ongoing Consultation and Support
Our team offers continuous virtual and in-person consultative services, including behavioral, occupational, physical, dietary, speech, recreational, and nursing consultations. These sessions help ensure care plans remain effective and responsive to changing needs.

- Requires referral and enrollment in SCS per state criteria
- Must be medically necessary and documented in the individual’s Person-Centered Plan (PCP).
What SCS Nursing Oversight Includes

Within 30 days of referral, a licensed nurse completes a full evaluation, including:
- Medication review
- Baseline vital signs
- Review of medical diagnoses
- Identification of nursing needs and caregiver training requirements
Monthly follow-ups ensure continuity of care and timely updates to the individual’s plan.
Individualized Care Plan

Each plan includes:
- Diagnosis-specific care instructions (for example, seizure or diabetes protocols)
- Clear delegation notes for Direct Support Professionals (DSPs)
- Monitoring plans for high-risk conditions
Weekly Medication Administration Record (MAR) Review

The nurse audits medication records for accuracy and compliance. Any discrepancies are corrected and documented for accountability.
